Treasury Inquest | Treasury urged to overhaul disciplinary procedure after employee's suicide

HM Treasury has vowed to update its disciplinary procedures following the death of Chloe Moffat, who had served as PA to Director General of International at the Treasury since 2022.

The inquest, first reported by the Guardian, heard Moffat was called into a meeting with her line manager and a senior manager - the Treasury's head of global issues - without prior notice of the meeting's subject or of the senior manager's attendance.

The inquest learnt she was told an anonymous complaint had been lodged, alleging she had shared confidential information about colleagues.

HM Treasury advised us the meeting was established as an early fact-finding meeting, and was not a formal disciplinary stage.
